Rehabilitation discharge is a critical and yet often times disorganized process. During the course of rehabilitation patients become accustomed to personal interactions in safe and structured environments with consistent care. Upon discharge, however, patients are often immediately faced with physical, financial and relational adversities that make critical decision making and follow through steps toward recovery a significant challenge.

 

 

At Triangle Wellness and Recovery, we aim to provide a more warm and connected transition into stable maintenance routines. Our providers, through telemedicine and face-to-face introductions, seek to begin building bridges of support and relationships with patients in real time prior to discharge. Through these interactions, patients can meet a friendly facilitator and can visualize their next steps with more confidence, knowing they will have continuity of care and adequate support to find safe relationships, spaces and routines beyond residential substance abuse treatment.

We encourage potential clients and referring providers to call our office directly at 919-338-5620. We offer a free phone or teletherapy consultation to potential clients. For individuals leaving a residential treatment setting, we collaborate with discharge planners and the client's treatment team to begin to build their therapeutic relationship with our practice before they move back to the Triangle area.
We are happy to talk more about our services and learn about your organization's specific niche and business needs. We strongly believe that patients can thrive when they have the best fit for treatment.
