Our providers, through telemedicine and face-to-face introductions, seek to begin building bridges of support and relationships with clients in real time prior to discharge. Through these interactions, clients can meet a friendly facilitator and can visualize their next steps with more confidence, knowing they will have continuity of care and support to move forward on their journey.
